PACE

The Program of All-Inclusive Care for the Elderly, or PACE, was established by Medicare to help independent seniors with complex medical needs. The program helps senior participants avoid nursing homes by allowing them to receive services necessary to their health and safety from the comfort of home while also having the opportunity to participate in group activities and receive personalized care.

1996 Founded

AltaMed has been a welcoming champion of PACE since 1996. In fact, AltaMed was California’s first licensed and accredited PACE program.

Today, the AltaMed PACE program consists of 11 centers, serving nearly 3,000 seniors each year.

About PACE

Isolation, loss of independence, and growing healthcare needs are often the overwhelming reality for our aging community members. Healthcare facilities and complete services that directly address all facets of senior patient health is critical for any community or family with an elderly loved one.

The Program of All-Inclusive Care for the Elderly, or PACE, was established by Medicare to help independent seniors with complex medical needs. The program helps senior participants avoid nursing homes by allowing them to receive services necessary to their health and safety from the comfort of home.

As California’s first licensed and accredited PACE program, AltaMed’s PACE program provides seniors with:

  • Clean, modern care facilities in the heart of the communities we serve
  • Independence and socialization through available daily transportation
  • Comprehensive health services with high standards all under one roof
  • Physical and occupational therapies for improved daily living
    Activities, meals, and nutritional counseling
  • Social services and home health care
  • iPads and tech assistance to help patients meet with doctors virtually and connect with other PACE patients
